A period of significant difficulties
It is clear that we are experiencing a period of significant difficulty for both doctors and their patients.
As I have written for much of the past 28 years, doctors face increasing—and quite intentional—pressure to abandon patients who do not show quick improvement.
Often, this pressure comes from healthcare insurance companies that see chronic patients with their often complex medical histories as loss leaders. Increasingly, it also stems from multiple US “healthcare agencies”: DHHS, CMS, CDC, FDA, and NIDA seem more focused on defending their healthcare fraud and scientific mistakes than on protecting those in pain.
Legislators often ask, “If there is a problem, then why aren’t we hearing more about it from patients?”

The answers are complex but often as straightforward as the nose on your face: you aren’t hearing because patients are increasingly dying from suicide, medical desertion, and their pain.

Surviving patients are often both disabled by pain and simply not knowledgeable enough to speak with authority, even if they weren’t terrified of losing what little pain care they still have if they speak up.
When patients and authorities remain silent, who is left? It leaves you, healthcare professionals of all kinds.
Only you have the vocabulary and professional stature to be heard if you speak out. Only you.
