The Tulsafication
of
Dr. Harold O. Alexander, MD

Regulatory racism, “economic lynching,” and “Tulsafication”
Regulatory racism is a term used by advocates and legal scholars to describe the systematic targeting, dismantling, and “burning down” of Black professional success.
In a medical context, this pattern is closely associated with concepts like “economic lynching” and “Tulsafication”—named after the historical destruction of the wealthy Black community of Greenwood in Tulsa. It represents the deliberate, administrative eradication of high-value Black achievement and intergenerational wealth because it succeeded in a system that has historically excluded or penalized such progress.

The Disproportionate Toll on Black Physicians
This pattern of exclusion is a documented systemic issue rather than a collection of isolated events. While African American physicians make up fewer than 5% of all doctors in the United States, investigative journalist Angela Green’s research reveals that hundreds of Black physicians are pushed out of practice every single year through administrative actions.

Minority practitioners frequently navigate an unlevel playing field characterized by individual, institutional, and structural inequities. These barriers, rooted in the legacy of slavery and reinforced by centuries of unequal treatment, manifest as deep disparities in healthcare, employment, and regulatory systems.

The Weaponization of Administrative Processes
Regulatory racism operates primarily through the administrative and disciplinary mechanisms of state medical boards and federal agencies. Because state boards wield immense power under Tenth Amendment state’s rights, and operate with a profound lack of oversight, transparency, and accountability, their disciplinary processes often become “end of the road” actions with no external checks.

Key tactics used to target independent Black physicians include:
- Forensic Weaponization of Medical Records: State medical boards routinely use medical record examinations as a “foot-in-the-door” to intrude into the sacrosanct physician-patient relationship. If a physician cannot comply with a threshold number of document submissions, or if minor clerical variances are found, boards aggregate these to manufacture charges and denigrate the doctor’s reputation.
- Subverting Clinical Standards: State boards often replace evidence-based medicine with a biased and loosely defined “preponderance of the evidence” to justify severe penalties. In Dr. Alexander’s case, his license was revoked following a safely performed, non-surgical outpatient procedure on a congenitally abnormal fetus—despite there being zero patient injuries, no patient complaints, and expert testimony confirming his clinical judgment met the standard of care. The board instead relied on hyperbolic risk claims and testimonies from physicians who had never actually performed the procedure themselves.
- Adversarial and Punitive Focus: Rather than seeking to support or rehabilitate practitioners, the relationship between regulatory boards and independent minority physicians is inherently adversarial. Boards use exhaustive investigations, tutorials, practice supervision, and random forensic testing to trap targeted providers in a cycle of administrative ensnarement.

The Broader Impact: Decapitation of Wealth and Community Care
The consequences of regulatory racism extend far beyond the individual physician, resulting in severe long-term damage:
- The Decapitation of Intergenerational Wealth: Forcing a highly trained specialist out of medicine permanently severs decades of accumulated clinical, surgical, and intellectual capital. This deprives the physician and their family of a lifetime of earnings, a practice, and a legacy, effectively dismantling the generation of Black wealth.
- Deprivation of “Race Concurrent Care”: When a Black physician is removed from an underserved area, the local community does not receive an identical replacement. Clinical research demonstrates that patients often achieve better health outcomes when treated by doctors who share their background. Removing these “providers of last resort” directly undermines healthcare equity and leaves marginalized patient populations vulnerable and underserved.
🛡️ Since Dr. Alexander emphasizes that state medical boards operate as adversarial entities, we could dive into the specific defensive and survival strategies he outlines for independent physicians currently facing board investigations.

Conclusion:
The Path to Resistance and Support
Dr. Alexander’s experience serves as a “word of caution” for the next generation. We must recognize that we are in a war of attrition, where the goal of the opposition is to exhaust the resources and resolve of the provider. For the pro-choice movement to withstand this Tulsafication, it must provide three critical pillars of support on an unrestricted basis:
- Logistic Support: To navigate the labyrinthine requirements of TRAP laws.
- Financial Support: To match the deep pockets of the radical right-wing foundations.
- Legal Support: Robust defense against the weaponization of medical records and the use of biased, non-expert testimony.
The struggle for reproductive justice requires us to look past the bureaucratic veneer of medical boards. We must remember that systemic dehumanization is often quiet and professional. As Dr. Alexander warns:

“Evil doesn’t snarl and have horns; it winks and wears a tie along the way.”
