DR. Harold Oliver Alexander, MD., OB-GYN

“…Sidelined with 45 years of accumulated medical and surgical intellectual capital by the Maryland medical board’s unjust revocation decision, I have decided to share my story…”

I am a 30-year American Board of Ob/Gyn Certified, native-born Washingtonian, and women’s reproductive rights advocate.
My more than 40-year odyssey began rather inauspiciously with my decision to believe and support the idea that a person was entitled to control their life’s destiny. Ethically, this principle is known as autonomy and is foundational to the concept of Ethics. I believed that the physician-patient relationship was sacrosanct and, therefore, not subject to interference. I was naive!

“…As a provider of last resort for the world’s largest vulnerable, undervalued, disenfranchised, and marginalized patient population, women…”

At 70 years of age, now out of practice for six years, and denied reinstatement of licensure, I would like to offer a word of caution to those physicians following in my footsteps.

Deprivation of one’s licensure is like being “blacklisted” and excluded from participation in a field that you’ve spent your life preparing for, much like what Colin Kaepernick went through in the NFL and Mahmoud Abdul Rauf in the NBA! You are excluded from practicing medicine nationally!!
Initially educated in the District of Columbia Public School System, I graduated from Frank W Ballou Senior High School; I attended Princeton U and Howard U undergraduate schools. After 3 years and two summers of premedical studies, I was admitted to Howard U College of Medicine, completing my MD at age 24 in 1977.

From there, it was on to internship and residency, split between Prince George’s General and Providence Hospitals in Cheverly, MD, and Washington, DC, respectively.
Following a stint in the USAF, where I finished as a Captain in the Medical Corp, I entered private practice in Ob/Gyn. I achieved American Board certification in 1986 and served as vice chairman and chairman of the Department of Ob/Gyn at Prince George’s General Hospital.

“…I BECAME A TARGET AND MIGHT AS WELL HAVE HAD A BULLSEYE ON MY BACK!..”

I was trained in all aspects of routine obstetrics and gynecologic surgery but had a passion for microsurgery, advanced laparoscopy, laser surgery, and reproductive gynecology, which became more of a staple with advancing age. Obstetrics requires much more stamina because of its inherent unpredictability.
As an African-American male I have had to contend with the same obstacles and unlevel playing field as the others in my race. Individual, institutional, and structural inequities, inequalities, and disparities that arose from slavery, amplified by 400 years of unequal treatment, racism, discrimination, injustices in healthcare, criminal and civil justice systems, and employment.

My experiences with women’s rights causes though, began during my early medical training while at the District of Columbia General Hospital and the Columbia Hospital for Women, where I was confronted with a myriad of requests for termination of pregnancy. I could neither perform them nor had I even decided whether I believed in abortion.

I consulted my mother first and, in rapid succession, each of my maternal and paternal aunts. With their guidance, I then prayed and made my decision to support a woman’s autonomy and right to control her reproductive system.
I have never been ambivalent, nor have I regretted my decision, despite becoming the object of extreme sociopolitical attention and injustice. I am just a collateral casualty of the ongoing war that women have fought since long before the Constitution was written, and they too like anyone who wasn’t “white and male” were disenfranchised.

As a provider of last resort for the world’s largest vulnerable, undervalued, disenfranchised, and marginalized patient population, women; I have found elements that are well organized, well funded, and clandestine interwoven into the fabric of existing legislative, judicial, and board processes.
Maybe I should have stated more properly that these elements have found me! Elements that, while operating in the shadows, have an agenda designed and dedicated to curtailing women’s reproductive rights to autonomy and choice by decapitating the process and removing the physician at any cost.

These pervasive elements yield outsized power because of wealth. One only needs to examine the case of Norma McCorvey(Jane Roe), who sold out the Pro-Choice movement only to recant her support of Pro-Life elements on her deathbed.

Right-wing organizations and foundations under the control of the Bradley, Freiss, Koch, Olin, and Scaife families espouse anti-choice, anti-woman, anti-family, anti-science, anti-evidence-based medicine, and anti-LGBTQ+ agendas.
I have seen women who have suffered from the horrors of unimaginable physical, psychological, emotional, and sexual abuse.
I was a Senior Sexual Assault Examiner throughout my residency years and beyond. Having examined and treated nuns, criminals, atheists, women of all religions, infants, children, men, LGBTQ, and other gender designations, all victims of sexual traumas.

“…EVIL DOESN’T SNAREL AND HAVE HORNS IT WINKS AND WEARS A TIE ALONG THE WAY…”

I have witnessed women making the gut-wrenching decision to terminate a desired pregnancy in the first, second, and third trimesters.
I have participated in the unsuccessful resuscitation efforts on a patient following a botched back-alley abortion. My decision to support autonomy and choice has saved women’s lives.

THE RISE OF STATE BOARDS AND FEDERAL AGENCY PRACTICES OF REGULATORY RACISM
During my practice of reproductive gynecology, I became one of a select few physicians who became specialists in the termination of pregnancies in all trimesters. I was the Medical Director of two area-wide Women’s Reproductive Services for ten years.

Despite my credentials, my licenses fell like dominoes, first ABOG certification, then the medical license, followed by federal and state DEA’s.
The final nail in the coffin was placed because of a safely performed outpatient, non-surgical, 3rd-trimester termination of a congenitally abnormal fetus.
No patient complaint, no morbidity or mortality issue, no true expert testimony from an abortion provider as to the State’s exaggerated, hyperbolic claims of patient risk from 2 Ob/Gyns who had never terminated a pregnancy; no physician impairment related to physical or mental illness, drug or alcohol use, and no moral turpitude. In addition, an expert in abortions provided testimony that there was no “abortion standard of care” violation

“…MARTIN LUTHER KING COULDN’T HAVE SAID IT ANY BETTER, “ IN THE END IT IS NOT THE WORDS OF OUR ENEMIES THAT WE WILL REMEMBER, BUT THE SILENCE OF OUR FRIENDS!..”
Lastly, but probably the most pervasively important factor, was the State’s overzealous, politically motivated weaponization of medical records.

There was an insatiable appetite to continuously examine my records, targeting My initial contact with the Board of Medicine, more than 15 years earlier, originated because of a patient undergoing an outpatient 2nd-trimester termination, who decided she had enough time to travel 3 hrs away(home) and then return to the office after being advised to remain in the vicinity of the office until completion of the procedure.

An out-of-state complaint was lodged when the patient appeared in the out-of-state ER and was picked up by a pro-life nurse.

The result was an allegation of patient dumping and a board request for examination of 25 records, in addition to the record of the involved patient. Medical records examinations are the tool that medical boards use to initially intrude into the doctor-patient relationship.

Medical records and documentation are weaponized. If you cannot comply with a threshold number of submissions, you are further investigated.
If deemed inadequate, medical records examinations are further used to denigrate and manufacture charges that result in disciplinary actions.

WHEN COMPLIANCE MEANS NOTHING

THE CONCEPT OF TULSAFICATION, THE CULTURE OF INDIFFERRENCE AND NON-ACCEPTANCE, HARLEM WISDOM TABLE
HEALTHCARE DISPARITIES IN EVERY MEDICAL CATEGORY
I am not a conspiracy theorist and have lived the experiences of being a “high-value target.” I was listed on the Nuremberg Files, an anti-abortion hit list compiled by Neal Horsley in the late 1990s.

I have been targeted by Operation Rescue, stigmatized, harassed, intimidated, defamed, slandered, coerced, character assassinated, condemned, picketed, received death threats, accosted, been audio and video surveilled, and had my neighborhood plastered with 3 ft by 4ft “Unwanted Posters.”
The Board of Medicine in Maryland has sought to solicit negative information from my patients, in my private office, and the public on its website, to no avail
Induced abortion is the most controversial, contentious topic in the USA and the world today. It is inextricably related to sociopolitical factors. One either believes in a woman’s right to privacy, autonomy, and choice or doesn’t. It’s either black or white. There are no grays.

I also know that any physician who has chosen to follow in my footsteps should heed my advice:
- Never TRUST that you will be treated fairly by any Board of Medicine or State Attorney’s Office.
- The relationship is always ADVERSARIAL. Their GOAL is to PUNISH you by some form of DISCIPLINARY ACTION. PRACTICE INVESTIGATION, TUTORIALS, PHYSICIAN REHABILITATION PROGRAMS THAT INCLUDE MONTHS OF RANDOM FORENSIC DRUG AND ALCOHOL TESTING, ADDICTIONOLOGY and NEUROPSYCHOLOGICAL EVALUATIONS, SUSPENSION, PRACTICE SUPERVISION, and REVOCATION. In the past 20 years, I have been through each.
- There may be SAFETY IN AFFILIATION. I didn’t affiliate with NAF, NARAL, or any other abortion support entity, though I cared for many referrals. When I needed support, I WAS LITERALLY OUT ON A LIMB!! ACLU and CENTER FOR REPRODUCTIVE RIGHTS also failed to respond to my dilemma.
- The price you may pay is LOSS OF YOUR LICENSE TO PRACTICE MEDICINE.
- BOARD’S FOLLOW ESTABLISHED QUOTAS ON THE NUMBER OF PHYSICIANS LICENSES TO BE REVOKED in collaboration with the FSMB(Federation of State Medical Boards). Lack of transparency prevents substantiation.
- ABORTION PROVIDERS ARE SPECIFICALLY TARGETED BY LAW(TRAP LAWS) Targeted Regulation of Abortion Providers. Medical records are weaponized against you!
- BEWARE THE “GESTAPO-LIKE” unannounced visits by board staff.
- REMEMBER, THEY CAN’T TAKE YOUR MD! ONLY GOD CAN DO THAT!
- STATE’S RIGHTS (via the 10 TH AMENDMENT) ALLOWS BOARD PROCESSES TO BE “END OF THE ROAD” PROCESSES FOR PHYSICIANS BECAUSE THEY LACK OVERSIGHT, TRANSPARENCY, and ACCOUNTABILITY!
THE TULSAFICATION OF DR. HAROLD O. ALEXANDER, MD, OB-GYN

I have been very fortunate in my life, to have had the opportunity to care for tens of thousands of patients in both outpatient and inpatient settings, done thousands of normal and complicated vaginal and operative deliveries, thousands of gynecologic surgeries, reproductive gynecologic surgeries and medical non-surgical procedures.

I have never performed any procedure that was outside the scope of my training, experience, or expertise or in an inappropriate setting.
My grandmother had a saying, “the proof is in the pudding”. For the lawyers, “res ipsa loquitur” speaks for itself. To have been challenged on the appropriateness of abortion care and the use of an abortifacient in a safely performed non-surgical third-trimester outpatient procedure is essentially being charged with malpractice.

Politics, targeting, prosecutorial discretionary targeting, together with prosecutorial advantage, disciplinary fervor, and biased expert testimony, can be combined to replace evidence-based medicine with a “so-called preponderance of the evidence. “ The result is invariably slanted toward Goliath and not David. In this case, Goliath wins because of an inherited power differential that skews the outcome. A sociopolitically created systemic unequal justice!

Unless there is a collective effort among the pro-choice elements to provide logistic, financial, and legal support on an unrestricted basis to abortion providers struggling in the quagmire of “trap laws”, this David versus Goliath scenario will continue to haunt women’s reproductive rights. The radical right-wing elements are precisely targeting physicians, and without physicians’ expert testimony in support of one another, unfortunately, their strategies will continue to prevail.
