Dr. Forest Tennant and his colleagues analyze a complex case of intractable pain
In this DocTalks episode, Dr. Forest Tennant and his colleagues analyze a complex case of intractable pain involving a patient who has developed a tolerance to fentanyl. The medical panel debates various treatment strategies, such as opioid rotation to methadone, hormone replacement, and the use of non-opioid adjuncts like DHEA.

A significant portion of the discussion features guest Dr. Harvey Jenkins, a retired orthopedic surgeon with a PhD in biochemistry, who shares his professional journey and his transition into pain management. Jenkins details his legal struggles with the state of Oklahoma, arguing that aggressive regulatory oversight and restrictive opioid policies have harmed both physicians and patients. He also introduces his new educational initiative, unbedsidemanners.com, which aims to improve scientific literacy among the public.

Finally, the speakers emphasize the life-threatening nature of untreated chronic pain and advocate for a risk-benefit approach to specialized medical care.

What is the ABC protocol for pain?
The ABC protocol, also known as the Anand Burrell Clement protocol, is a pharmacological strategy for treating severe, intractable pain that combines three classes of medication to achieve a synergistic effect.

According to the source, the protocol consists of:
- A – Narcotic Analgesics: Such as methadone, which is often preferred in difficult cases due to its unique mechanism of action with the NMDA receptor,.
- B – Benzodiazepines (Anti-anxiety medications): These are included to address anxiety and help patients achieve necessary sleep,.
- C – Muscle Relaxants: Examples mentioned include baclofen (referred to as “blein”) or tizanidine (“taanadine”).
Key Insights into the Protocol:
Alternative Support: While the ABC protocol focuses on these three categories, clinicians also suggest supplementing it with the “Tenant Protocol,” which involves hormonal support with DHEA (Dehydroepiandrosterone ), pregnenolone, and PEA (Palmitoylethanolamide).
Synergy over Monotherapy: The primary goal of the ABC protocol is for the drugs to work together more effectively than any of them would individually.
Medical vs. Street Terminology: The source notes that this combination is sometimes pejoratively referred to as the “Holy Trinity” by law enforcement or in street terms,. However, the clinicians in the source reject this name, arguing it is misleading and that the term is often used by prosecutors to criminalize what is actually an important medical procedure.

How do hormone panels improve pain treatment?
Hormone panels improve pain treatment by identifying physiological deficiencies that can render standard pain medications ineffective. According to the sources, severe and intractable pain is a life-threatening condition that can throw a patient’s metabolism off and cause their hormone levels to drop significantly.

Specifically, hormone panels help treatment in the following ways:
- Restoring Medication Efficacy: A critical insight shared by Dr. Tennant is that if key hormones like testosterone, pregnenolone, or DHEA (DHCA) are severely depleted, opioid medications may stop working. Identifying and correcting these deficiencies is necessary for the primary pain treatment to be successful.
- Providing Early Effectiveness: Supplementing hormones, such as testosterone, can help a patient achieve “really early effectiveness” from their prescribed pain relievers.
- Guiding Specific Protocols: Results from these panels allow clinicians to implement targeted hormonal support, often referred to in the sources as the “tenant protocol” or the “PA DHEA pregnenolone protocol”. This approach involves using substances like DHEA, pregnenolone, and PEA to support the patient’s physiological ability to manage pain.
In summary, hormone panels transition pain management from simply masking symptoms to addressing the biochemical and metabolic disruptions caused by chronic pain, ensuring that the body is capable of responding to analgesic therapy.

HOW FLAWED AI CREATES INJUSTICE
“It is also questionable whether clinical reliance on PDMP risk scores is truly voluntary, given that the regulatory environment leaves providers with little choice but to take seriously the information generated by proprietary PDMP algorithms.”

This creates a conflict for providers, who may be caught between their own expert clinical judgment about a patient’s needs and the demands of a surveillance system they are compelled to use. This shifts the locus of medical authority from the provider’s professional judgment to the algorithm’s opaque and unsubstantiated output.

Norman,
We’ve spoke on the phone about a few items addressing the doctors defense for Red Lawhern and other law firms to have and use at will.
I read Dr Harvey is in Oklahoma, I have so much I want to share with him directly about how I’ve been treated, all the dismissal letters saying i’m to complex the loss of employment due to discrimination and how my husband went with me to one appointment and the doctor fled the room saying “he feared for his life” that was a bogus claim it was to push this doctors procedures and force OUD on me that is not necessary.
They have shut down a few pain clinics here and I sure hope it’s because of his work in the really bad state of #1 in medical malpractice lawsuits (you should read them, they are frightening)
Anyway, If i can offer anything to Dr Harvey to show this I kept everything, I’ve wrote letters to HHS and filed complaints and only received canned replies or nothing at all.
If this is true, I’d like to share all this with him and help anyway I can.
Dr Isben has been my saving grace, along with APDF foundation and now PART Texas, and I’m able to do more now than before so I want to help.
Thank you for keeping us posted on all events and news, I count on it.
Thanks again,
Deann (Goudy) Bartolotta
