OPIATE ADDICTION The Pathophysiology Of A Chronic Disease
It was recently reported that Suboxone has been greatly underutilized. I wrote about this in my unpublished paper in 2016 “Opiate Addiction The Pathophysiology Of A Chronic Disease “. I treated opiate addicted individuals for more than 11 years. I did it based on several medical facts that I learned about and believed in.
None of these facts have been widely pushed by the medical community or the general public. The emphasis has been on blaming someone or somebody for the opiate crisis. No one wants to admit that you can’t cure opiate addiction. No one wants to admit that you can’t stop illegal drugs from entering the country. No one wants to admit that the “War On Drugs “ is a total failure and new methods must be instituted.


First our emphasis should be on reducing overdose deaths not addiction . Stop the blame game and recognize that the physiological action of opiates is determined by opiate receptors and metabolism not pharmaceutical companies and health care providers.

In determining treatment for pain realize that most patients who present to a physician for treatment have already tried and failed the alternative treatments recommended before opiate pain medication is started. Realize that addiction from opiate pain medication affects only 1 percent of patients treated with opiate pain medication.
Once addiction is identified there are two parallel treatment options that should be instituted.

The first is the use of Medication Assistance Treatment ( MAT). This should be done with Suboxone or Sublocade. For those few who are allergic to Suboxone or Sublocade Methdone at its lowest dose is an alternative.
The second treatment is a mitigated technique designed specifically to reduce overdose deaths from illegal substances. This technique has been embraced by many countries and has had a tremendous impact on reducing overdose deaths.
Those individuals addicted to Heroin are given Heroin. Those individuals addicted to opiate pain medication are given opiate pain medication. Safe injection sites and the ability to test for fentanyl has also reduced overdose deaths. These commen sense approaches will cause an immediate reduction in overdose deaths. Long turn clinical trials to determine best practices should be instituted.

MESSAGE FOR PENNSYLVANIA GOVERNOR JOSH SHAPIRO
On June 27 2022 the United States Supreme Court ruled 9-0 that a healthcare provider could not be charged with a crime for prescribing a controlled medication they were authorized to prescribe. In spite of this ruling I and other health care providers have not had our sentences vacated. As Governor you have the authority to vacate those sentences.
As the former Attorney General of Pennsylvania your office was responsible for the conviction of these innocent health care providers. I am asking that you immediately vacate these sentences and restore these falsely acused individuals to their precharged condition.
